We recognize there might be instances when an individual may have a personal preference to report grievance complaints and not disclose his/her identity. This form provides the opportunity to do so. However, the PIOJ will not be able to follow-up or discuss options for resolution without the name of the complainant. Please note that anonymous complainants can change their mind at any time, and come forward with identifying information.
